You must be 18 years old or older to sign this document. The cost of shipping, the kennel crate and health certificate is extra, and the responsibility of the purchaser.

1) PWD has been seen by a licensed DVM no less than three weeks before being sold. PWD is guaranteed to be healthy at time of sale. Tenmen recommends that the buyer has dog seen by DVM within one week of purchase. If, within one week; buyers DVM certifies that dog is not healthy then Tenmen will refund purchase price minus any shipping/delivery costs.

2) If the dog herein is affected by Storage disease (as certified by DVMs necropsy report) or PRA (as certified by a Canine Ophthalmologist) a replacement puppy will be given from next litter or cash refund will be given on receipt of report.

3) Buyer agrees to have the dog tested for Storage Disease, x-rayed for hip dysphasia, optigen tested and eyes checked by a canine ophthalmologist before using this animal for breeding. Buyer also understands that Tenmen recommends this as normal care-taking of animal regardless of intention on buyer's part to breed this animal.

1) Buyer understands and agrees to adequate and reasonable care to maintain health and well being of this dog. Buyer also agrees to notify Tenmen should any health problems arise.

2) Tenmen has first option to get dog back if buyer decides to transfer ownership of this dog for any reason. If Tenmen can't take the dog back, they would like to be active in placing the dog.

3) Buyer agrees to NEVER sell, give, or transfer ownership of this dog to puppy mills, federal wholesalers, pet store, or outlets and for chemical research.

4) Buyer agrees to let Tenmen see this dog or a picture of this dog when dog is 4-6 months, 6-9 months 9 months/1 yr of age and receive any copies of title winning pictures for display on our web site or publication.

1) AKC registration application given 2 weeks after the transfer of the dog to the buyer, unless spay/neuter agreement is entered; then application is given after the DVMs certificate indicating date of surgery is sent to Tenmen.

2) We will also furnish a complete shot record and medication sheet for any medicine or shots given to your puppy. We are not responsible for any person or animal bitten or hurt by the animals we sell. The training and behavior of animals after we sell and transfer ownership, is the responsibility of the new owner.

3) Buyer is required to use name TENMEN'S _________________________ as registered name, but understands that they may call dog whatever they wish.
